SPORTS PHYSICAL INFORMATION

Dear Parents/Guardians:
The State Department of Education has revised the physical policy. The following is quoted from the Student Health Services Guidelines, which can be found here.
"Each student medical examination must be conducted by a healthcare provider or advanced practice nurse chosen by the student's parent/guardian at the provider's facility (the student's "medical home"), and a full report of the examination - documented on an approved school district form, dated, and signed by the medical provider - must be presented to the school."
This essentially means that the high school can no longer provide physicals to prospective student-athletes. These individuals need to follow the guidelines above in order to receive clearance for participation.
Physicals need to be signed and stamped by the medical doctor. The physical can not be completed by a chiropractor.
The above change in code is being emphasized at this time in order to ensure that all student athletes have their physicals completed in plenty of time to participate in the spring season.
