The following guidelines are from the policy of the Vernon Township School District and the NJ State Board of Education.
- Your child must have a physical exam that was done after July 1st, 2007. Once on file, the physical is good for the entire school year. Your child may not participate (tryout or practice) until this physical has been handed in to the coach or school nurse. THERE WILL BE NO EXCEPTIONS.
- Your physician must complete ALL PORTIONS OF THE FORM, sign, and apply his/her stamp. We cannot accept a form without both a signature and a stamp or with any incomplete areas. WE CANNOT ACCEPT A PHYSICAL EXAM ON ANY FORM OTHER THAN THE SCHOOL FORM. The form will be returned to you if it is not completed and your child will not be able to participate until it is returned to the coach or school nurse.
- We cannot accept a physical exam performed by a chiropractor.
