PLEASE READ THE FOLLOWING INSTRUCTIONS REGARDING THE SPORTS PARTICIPATION FORMS.
These guidelines are from the policy of the Vernon Township School District and the NJ State Department of Education.
- Your child must have a physical exam that was completed within 365 days of the date of the first practice. Once on file, the physical is good for one year from the date the exam was administered but a new physical packet (PART A & PART B) must be completed for each school year. YOUR CHILD MAY NOT PARTICIPATE UNTIL ALL FORMS HAVE BEEN HANDED IN TO THE COACH OR SCHOOL NURSE.
- Please note: Your physician must complete ALL PORTIONS OF THE FORM, SIGN AND APPLY HIS/HER STAMP. We cannot accept a form without both the signature and the stamp or with any incomplete areas. WE CANNOT ACCEPT A PHYSICAL EXAM ON ANY FORM OTHER THAN THE STATE-ISSUED FORM. The form will be returned to you if it is not completed and YOUR CHILD WILL NOT BE ABLE TO PARTICIPATE until the correct form is handed in.
- Please be sure that PART A-HEALTH HISTORY QUESTIONNAIRE is completed by a parent /guardian and taken with the PART B- PHYSICAL EVALUATION FORMS to your doctor for his/her review.
- We CANNOT accept a physical exam performed by a chiropractor.
- If you have any questions, please contact the school nurse at your child’s school or the athletic department if the nurse is unavailable at 973-764-2900 ext. 5742.
